will immediately provide you with the information needed to receive
the price stated below for your hotel accommodations (if needed).
To Register

Send
your completed registration form along with a check for the appropriate
fee (payable to: FMT Solutions, Inc.) to our Oregon
office.
Send
a money order or cashiers check only if checks are received under
1 week away from Training Conference.
When
we receive your payment, we will provide you with the information
needed to receive the price stated below for your hotel accommodations
(if needed).
Travel
and Hotel:
As an attendee,
you must arrange and pay for any travel or hotel accommodations.
FMT
Solutions has special room rates available at the Holiday
Inn.
Discounted
room rate for Thursday, March 31, 2005 is $90 (single or double occupancy).
Hotel
room rates are subject to applicable state and local taxes.
If you
are flying in for the training conference:
Schedule
your flight to arrive at Chicago O'Hare International Airport (ORD)
which is 3 miles from the hotel.
Complimentary
shuttle is available from and to O'Hare Airport.
