How to Register:

Seating is limited. Register early!

Registration Fees:

  • $299 per person before Feb. 25, 2005
  • $399 per person before Mar. 18, 2005
  • $599 per person after Mar. 18, 2005

Registration includes breakfast, lunch, and training materials.

To Register and Pay by Credit Card:

  • Contact us via phone to pay by credit card.
  • Provide us with your registration information over the phone or complete the registration form at the bottom of this page.
  • We will immediately provide you with the information needed to receive the price stated below for your hotel accommodations (if needed).

To Register and Pay by Check:

  • Complete the registration form at the bottom of this page.
  • Send your completed registration form along with a check for the appropriate fee (payable to: FMT Solutions, Inc.) to our Oregon office.
  • Send a money order or cashiers check only if checks are received under 1 week away from Training Conference.
  • When we receive your payment, we will provide you with the information needed to receive the price stated below for your hotel accommodations (if needed).

Travel and Hotel:

As an attendee, you must arrange and pay for any travel or hotel accommodations.

  • FMT Solutions has special room rates available at the Holiday Inn.
  • Discounted room rate for Thursday, March 31, 2005 is $90 (single or double occupancy).
  • Hotel room rates are subject to applicable state and local taxes.

If you are flying in for the training conference:

  • Schedule your flight to arrive at Chicago O'Hare International Airport (ORD) which is 3 miles from the hotel.
  • Complimentary shuttle is available from and to O'Hare Airport.

If you are traveling by car:

  • The Holiday Inn Chicago-O'Hare/Kennedy is located at 8201 West Higgins Rd., Chicago, IL 60631.
  • Self-parking is complimentary at the hotel for this event.

Cancellation Policy:

  • If you are unable to attend, you may send a representative in your place.
  • If you cancel before March 18, 2005 you will receive a refund minus a $150 cancellation fee.
  • No shows are not eligible for a refund.
